吻合器痔上黏膜环形切除术治疗高龄患者Ⅲ～Ⅳ期内痔69例临床观察

Clinical observation of procedure for prolapse and hemorrhoids for treating stage Ⅲ-Ⅳ internal haemorrhoids in 69 senile patients.

目的观察使用吻合器痔上黏膜环切术治疗高龄患者Ⅲ～Ⅳ期内痔临床效果。方法回顾性分析69例采用吻合器痔上黏膜环切术治疗高龄Ⅲ～Ⅳ期内痔患者的临床资料，选取同期采用传统手术治疗病例46例作为对照，比较分析两组患者的手术时间、住院天数、术后疼痛、术后感染以及术后水肿消退时间。结果吻合器痔上黏膜环切术患者手术时间、住院天数、术后疼痛、术后感染以及术后水肿消退时间等指标优于传统手术患者，差异有统计学意义（P〈0．05）。结论吻合器痔上黏膜环切术是治疗高龄内痔患者的有效方法。

Objective To evaluate the therapeutic effect of senile patients with stage Ⅲ-Ⅳ internal haemorrhoids treated by procedure for prolapse and hemorrhoids. Methods The clinical records of 69 senile patients （the stud group） who underwent the procedure for prolapse and hemorrhoids were reviewed retrospectively. Forty-six pa- tients treated by condition surgery methods at the same period were selected as the control group. The operation time, length of hospital stay, patients with postoperative pain and infection, and edema disappearance time were compared between the two groups. Results In the study group, the average operation time, the length of hospital stay and ede- ma disappearance time, the patients with postoperative pain and infection were （19.7±4.7） minutes, （4.3±2.4） days, （2.42±0.65） days, respectively, eighty cases and one. In the control group, those indexes were （53.2±5.3） minutes, （10.2± 3.1） days, （6.22± 1.75） days, thirty eases and six cases, respectively. There were statistically significant difference between the two groups in these indexes, all P〈0.05. Conclusion The procedure for prolapse and hemor- rhoids is an effective approach in the treatment of senile patients with stage Ⅲ-Ⅳ intemal haemorrhoids.
