贲门癌侵犯胃裸区的CT、MRI表现及临床意义

CT and MRI manifestation and clinical significance of gastric bare involvement by carcinoma of gastric cardia

目的探讨贲门癌侵犯胃裸区的CT、MRI表现及其相关的病理解剖学基础。方法分析手术病理证实45例贲门癌的CT与MRI影像学资料。结果 45例行CT检查,其中贲门癌侵犯GBA患者26例,高达57.8%。贲门癌侵犯GBA的CT表现分两大类,贲门癌直接侵犯GBA形成肿块21例(80.8%),转移性淋巴结侵犯GBA5例(19.2%)。11例行MR检查,其中5例(45.5%)侵犯GBA,4例肿块直接侵犯GBA,淋巴结转移1例。结论贲门癌侵犯GBA有一定的CT及MRI表现特征和解剖基础,术前检查评价贲门癌侵犯GBA有重要临床意义。

Objective To approach CT and MRI manifestation of carcinoma of gastric cardia with bare area involvement and the correlating pathological anatomy. Methods Manifestation of CT and MRI for the demonstration of 45 cases of carcinoma of gastric cardia were evaluated with surgical proof and pathological diagnosis. Results There were 26 cases with bare area involvement （57.8%） in 45 cases of carcinoma of gastric cardia. CT features of carcinoma of gastric cardia with GBA involvement include two parts. The GBA of carcinoma of gastric cardia were encroached by tumors. The soft mass was found in 21cases （80.8%）. 5 cases were metastatic lymph nodes infringement GBA （19.2%）.MRI underwent in 11 cases, and 5 cases with GBA involvement, including 4 cases of tumor erosion directly and 1 case of lymph node matastasis. Conclusion GBA involvement by carcinom of gastric cardia shows some characteristic CT MRI signs and anatomic-pathological basement. It is important that the diagnosis of GBA involvement should be done before operation.
