高血压患者工作场所干预依从性的影响因素 被引量:2

Factors affecting therapeutic compliance in hypertensive patients in workplace

摘要 目的探讨高血压患者工作场所干预依从性的影响因素。方法 2009-04-2010-05开始对开滦总医院井下及井下辅助单位在岗原发性高血压职工进行综合干预。干预期间每两周随访1次并测量血压,根据研究对象的依从性分为依从组和非依从组。分析依从性的影响因素。结果在所有研究队列中依从组1964例,占48.0%,非依从组2126例,占52.0%;单因素Logistic回归分析显示与研究对象依从性相关的因素有性别、年龄、工种、随访中加量加药、换药、教育程度、基线收缩压、基线舒张压和行政干预;多因素Logistic回归分析显示影响研究对象依从性的因素有工种、教育程度、基线收缩压及行政干预,OR值分别为0.645、0.567、1.245和7.670。结论井下作业、教育程度低、基线收缩压低及行政干预措施差是高血压患者工作场所干预依从性差的影响因素。 Objective To explore the factors affecting therapeutic compliance in hypertensive patients in workplace. Methods The workers with hypertension received comprehensive intervention for hypertension from April 2009 to May 2010. The patients were either underground or assistant workers in coal mine from Kailuan Group. During the intervention period, patients' blood pressure were measured every two weeks. The patients were divided into two groups based on their compliance. The effecting factors on patients' compliance to intervention were analyzed. Results There were 1964 patients (48.0% } in compliance and 2126 patients (52.0~) in non-compliance group. Single Logistic regression analysis showed that gender, age, work type, dose increase, changes o{ treatment regi- ment, education, baseline blood pressure, and administrative intervention were influencing factors for compliance. Multiple Logistic regression analysis showed that work type, education, baseline systolic blood pressure { SBP} and administrative intervention were influencing factors for compliance (OR 0. 645, 0. 567, I. 245 and 7. 670, respectively). Conclusion Underground workers, tow level of education, baseUne SBP, and poor administrative intervention were factors affecting compliance in hypertensive patients in the workplace.
