高龄脑出血微创手术治疗探讨

Minimally invasive surgery for elderly patients with cerebral hemorrhage

目的探讨微创手术在高龄高血压脑出血患者治疗中较保守治疗方法的优点和疗效。方法 94例70岁以上高血压脑出血病例,按照治疗方案不同分为2组,微创治疗组(46例)采用钻孔置管手术,保守治疗组(48例)未采取手术治疗。比较两组患者疗效及并发症。结果微创手术组并发症发生率为43.5%(20/46),远低于保守治疗组并发症[79.2%(38/48),P<0.05]。1个月后微创组患者病死率[13.04%(6/46)]也明显低于传统治疗组[33.33%(16/48),P<0.05]。结论微创手术治疗高龄高血压脑出血可明显降低患者的并发症发生率和病死率,改善患者预后。

Objective To evaluate the application of minimally invasive surgery for elderly patients with hypertensive intracerebral hemorrhage. Methods Ninety four patients with hypertensive intracerebral hemorrhage aged over 70 years were assigned to receive minimally invasive surgery （n = 46） or conservative treatment （n = 48 ）. The efficacy and complications were compared between two groups. Results The complication rates in minimally invasive surgery group and conservative treatment group were 43.5% （20/46） and 79.2% （38/48）, respectively （P 〈0.05）. The mortality rates after 1 month in minimally invasive group and conservative treatment group were 13.04% （6/46） and 33.33% （16/48）, respectively （P 〈0.05）. Conclusion Minimally invasive surgical treatment for elderly patients with hypertensive cerebral hemorrhage can significantly reduce the incidence of complications and mortality.
