应用DATEXUltima监测仪观察CO_2气腹对机体呼吸功能和氧耗的影响

摘要 目的 :通过 Ultima监测仪观察 CO2 气腹对机体呼吸和氧耗有何影响。方法 :在气管内静吸全麻复合硬膜外麻醉下行 L C。以 Ultima监测仪持续监测呼吸功能和气体的变化。结果 :PETCO2 和 VCO2 在气腹 2 0 m in后较气腹前显著增加。 PAW在气腹开始后较气腹前显著升高 (P<0 .0 0 1)。 MVE在气腹后较气腹前降低 (P>0 .0 5 )。 VO2 在气腹后 10min时显著高于气腹前水平 (P<0 .0 1) ,以后继续升高。结论 :在 CO2 气腹期间 ,通过 Ultima监测仪能连续观察 PETCO2 、VT、MVE、PAW及 VO2 的动态变化。 Objective: To investigate the effects of intraperitoneal CO 2 insufflation on respiration and oxygen consumption by using DATEX Ultima monitor during LC. Methods: Maintenance of anesthesia used N 2O,continous intravenous infusion of propofol and intermittent epidural administration during LC.Respiratory dynamics and concentrations of respiratory gases were monitored with DATEX Ultima monitor.Results: P ET CO 2 ,MV E ,VCO 2 and VO 2 before CO 2 insufflation were significantly lower than that before induction of anesthesia.P ET CO 2 and VCO 2 markedly increased during 20~60 min after CO 2 insufflation, compared with that before CO 2 insufflation.P AW remarkably rose and MV E markedly decreased during early CO 2 insufflation.VO 2 at 10min after CO 2 insufflation was evidently higher than that before CO 2 insufflation and then continued increasing. Conculsions: During CO 2 insufflation,we can observe continously dynamic changes of P ET CO 2,VT,MV E ,P AW and VO 2 by DATEX Ultima monitor.
