摘要
目的探讨七氟醚预处理对大鼠肝缺血-再灌注后心肌损伤的影响及可能机制。方法健康雄性SD大鼠30只,随机分为假手术组(Sham组,n=10)、肝缺血-再灌注组(IR组,n=10)和七氟醚预处理组(S组,n=10)三组。建立大鼠肝缺血-再灌注模型,S组大鼠在阻断入肝血流前吸入2.5%七氟醚30min,IR组和S组大鼠肝缺血45min再灌注120min。分别测定各组大鼠血清谷丙转氨酶(ALT)、谷草转氨酶(AST)、心肌型肌酸激酶(CK—MB)、乳酸脱氢酶1(LDH1)浓度及心肌组织丙二醛(MDA)、超氧化物歧化酶(SOD)、肿瘤坏死因子(TNF-α)、白细胞介素6(IL-6)浓度,光镜观察心肌组织形态学。结果与Sham组比较,S组和IR组血清ALT、AST、CK—MB、LDH1浓度均明显升高(P〈0.05);心肌组织MDA、TNF—α和IL-6浓度明显升高(P〈0.05),SOD活性明显降低(P〈0.05)。与IR组比较,S组血清ALT、AST、CK—MB、LDH1浓度明显降低(P〈0.05);心肌组织MDA、TNF-α和IL-6浓度明显降低(P〈0.05),SOD活性明显升高(P〈0.05)。Sham组心肌肌束及细胞形态正常,IR组和S组可见肌束断裂、细胞边界不清及炎症细胞浸润,S组改变较IR组轻微。结论七氟醚预处理可减轻大鼠肝缺血-再灌注后心肌损伤,其作用机制可能与减少氧自由基的释放和抑制炎症反应有关。
Objective To investigate the effect and possible mechanism of sevoflurane preconditioning on the injury of myocardium after hepatic ischemia- reperfusion(IR) in rats. Methods Thirty adult male Sprague -Dawley (SD) rats were randomly divided into three groups: sham operation (sham group ), ischemia- reperfusion (IR group ) and sevoflurane preconditioning (S group). Hepatic IR models of the rats were established. In S group, the rats were pretreated with 2.5% sevoflurane for 30 minutes before IR. IR group and S group, the ischemia time of rat liver was 45 min and reperfusion 120 min. The content of ALT, AST, CK - MB, LDH1 in serum and MDA, SOD, TNF - α, IL - 6 in myocardium of all groups were detected. Myocardial tissue morphology was observed by light microscope. Results Compared with sham group, the content of ALT, AST, CK - MB, LDH1 in serum and MDA, TNF- α, IL- 6 in myocardium increased significantly, while SOD activity decreased significantly in IR and S group ( P 〈 0.05 ). Compared with IR group, the content of ALT, AST, CK-MB, LDH1 in serum and MDA, TNF- α, IL-6 in myocardium decreased, and the SOD activity increased in S group (P 〈 0.05 ). In sham group, myocardial muscle bundles and cell morphology were normal. In IR group and S group, muscle bundles were broken, cell boundary was unclear and inflammatory cell infiltration was visible, and S group slightly changed. ConclusionSevoflurane preconditioning can attenuate the injury of myocardium following hepatic IR in rats, and possible mechanisms are associated with the decrease of oxygen free radicals and the inhibition of the inflammatory response.
