108例外阴阴道假丝酵母菌病菌种分型及药敏研究 被引量:2

摘要 目的了解外阴阴道假丝酵母菌病(VVC)及复发性外阴阴道假丝酵母菌病(RVVC)的菌种分布情况及药物敏感性。方法收集28例具有典型临床症状的RVVC患者及80例VVC患者的阴道分泌物标本,用沙堡氏琼脂培养基培养。菌种鉴定采用ATBExpression半自动细菌鉴定仪,药物敏感性测定使用真菌药敏纸片。结果108株假丝酵母菌中,白色假丝酵母菌VVC组和RVVC组分别占90.0%和71.4%;非白色假丝酵母菌RV.VC组和VVC组分别占28.6%和10.0%;RVVC组白色假丝酵母菌比例低于VVC组,差异有统计学意义;两组中非白色假丝酵母菌均以光滑假丝酵母菌为主。RVVC及VVC组对抗真菌药物的敏感性依次为制霉菌素(99.1%)〉克霉唑(95.4%)〉酮康唑(94.4%)〉咪康唑(71.3%),白色假丝酵母菌对唑类药物的敏感性高于非白色假丝酵母菌,差异有统计学意义。结论白色假丝酵母菌是RVVC、VVC的主要菌种,非白色假丝酵母菌比例在RVVC组显著高于VVC,其中以光滑假丝酵母菌为主;假丝酵母菌对制霉菌素,酮康唑,克霉唑的敏感性高,对咪康唑的敏感性低,且非白色假丝酵母菌对咪康唑的敏感性显著低于白色假丝酵母菌。 Objective To characterize the species of candida and drug susceptibility in patients with recurrent vulvovaginal candidiasis (RVVC) and with vulvovaginal candidiasis (VVC). Methods From November 2007 to March 2009, 28 RVVC cases and 80 VVC cases were enrolled in this study. Fungal culture and drug suseeptibiUty tests of vaginal secretion were performed. Results The ratio of candida albicans was 90.0 % in VVC group (72/80) compared with 71.4 % in RVVC group (20/28). The ratio of non-albieans candida was 28.6 % in RVVC group (8/28) compared with 10.0 % in VVC group (8/80). There was a significant difference between the ratios of non-albicans candida in RVVC and in VVC (P 〈 0.05). The dominant non-albicans was candida glabrata in the two groups. Drug susceptibility test showed that 99.1% of these patients were sensitive to nystain, 95.37 % to clotrimazole, 94.44 % to ketoconazole, and 71.3 % to miconazole. Drug sensitivity of candida albicans to azole was higher than of non-albicans candida. There was a significant difference between candida albicans and non-albicans candida to miconazole. Conclusion Candida albicans was the dominant pathogenic candida in VVC and RVVC patients, with a higher proportion of non-albieans candida in RVVC than in VVC patients. Candida glabrata was the dominant non-albicans candida. Candida albicans was sensitive to nystain, clotrimazole, and ketoconazol, but less sensitive to miconazole.
