小儿喉罩置入时七氟烷和丙泊酚合适浓度临床应用比较 被引量:2

摘要 目的观察小儿麻醉时七氟烷吸入、丙泊酚TCI在喉罩置入时的最适浓度的临床疗效。方法择期腹股沟斜疝手术60例,年龄1~11岁,ASAⅠ-Ⅱ级,体重9~32kg。基础麻醉采用东莨菪碱0.01mg/kg+氯胺酮2mg/kg臀部肌内注射。七氟烷组(A组,n=30)采用七氟烷吸入,无基础麻醉。丙泊酚组(B组,n=30),采用效应室浓度靶控输注,有基础麻醉。根据Dixon序贯法来确定喉罩置入时的合适浓度。在咽喉反射消失、下颌松弛、睫毛反射消失时用反转法置入。观察入室时、喉罩置入前和置入后的心率、呼吸频率、PETCO2、SPO2及术中不良反应。结果 2组均达到了麻醉需求浓度。麻醉效果均较好。结论已行基础麻醉小儿适用丙泊酚靶控输注置入喉罩麻醉,而未做基础麻醉的小儿则用七氟烷吸入诱导后置入喉罩较为合适。 Objective To observe when pediatric anesthesia seven fluorine inhalation,c) poisson phenol TCI laryngeal mask placement in the optimal concentration of the clinical curative effect.Methods 60 patients undergoing elective groin surgery,age 1~11 years old,ASA Ⅰ-Ⅱ level,weight 9~32 kg.Don’t preoperative drug use,the conventional fast the drink.The 0.02 mg based anesthesia atropine/kg + ketamine 2 mg/kg coxal muscle injection.Seven fluorine silane group(group A,n = 30) using seven fluorine silane inhaled,no basic anesthesia.C poisson phenolic group(group B,n = 30),use effect room concentration target control infusion,have basic anesthesia.According to the method to determine the Dixon sequential laryngeal mask placement of the appropriate concentration.In the throat reflection to disappear,jaw relaxation,eyelash is reflected in reverse disappeared into the law.Watch home invasion,laryngeal mask when put into the heart rate before and after,breathing rate,PETCO2,SPO2 and perioperative adverse reactions.Results The two groups have reached anesthesia demand concentration.Anesthesia effect were well.Conclusion C poisson phenol target control into the infusion of laryngeal mask more suitable to the already do basic pediatric anesthesia,and did not do basic anesthesia with seven children is inhaled after induction into the silane fluorine laryngeal mask relatively appropriate.
