门脉高压急性大出血贲门周围血管离断术54例分析

Portal hypertension with acute massive hemorrhage of pericardial devascularization with analysis of 54 cases

目的总结门脉高压急性大出血行急诊贲门周围血管离断术的治疗体会。方法回顾性总结54例门脉高压症并食管胃底静脉曲张破裂大出血患者临床资料,均急诊行脾切除,贲门周围血管离断术。结果 54例均获得随访,近期死亡3例,其余51例随访6个月至7年,无再出血,死亡8例,余43例疗效满意。结论注重围手术期合并症的处理,合理选择适应证,精细操作,确切断流,急诊手术是治疗门脉高压症食管胃底静脉曲张破裂出血的有效措施。

Objective To summarize the acute massive hemorrhage of portal hypertension emergency perieardial devascularization for the treatment of. Methods Retrospective analysis of 54. eases of portal hyper-tension esophageal and gastric variceal bleeding in patients with clinical data, are emergency splenectomy, peri-cardial devascularization. Results 54 cases were followed up,3 cases of recent death, the remaining 51 cases were followed up for 6 months to 7 years, no bleeding,8 cases died,43 cases with satisfactory results. Conclu- sion Pay attention to surround operation period treatment of complications, the reasonable choice of indica-tions, fine operation, exact stop, emergency operation for portal hypertension esophageal variceal bleeding and effective measures.
