摘要 探讨 P—VEP在蝶鞍区肿瘤诊断中的价值。方法 :对 2 0例蝶鞍区肿瘤患者作 P—VEP检测 ,记录 60、1 5min黑白方格刺激的 P— VEP中 N75 、P1 0 0 、N1 3 5 振幅及潜时。结果 :1 5min P1 0 0 异常占 95% (38/4 0 ) ,其中振幅降低占 50 % (2 0 / 4 0 ) ,潜时延长 7.5% (3/ 4 0 ) ,潜时振幅均异常占 37.5% (1 5/ 4 0 )。1 5min P1 0 0 波形异常 70 % (2 8/ 4 0 ) ,其中消失波 2 2 .5% (9/ 4 0 ) ,W波或 U波占 47.5% (1 9/ 4 0 ) ,正常波占 30 % (1 2 / 4 0 )。结论 :蝶鞍区肿瘤的 P—VEP检测异常率高 ,以 1 5min P1 0 0 振幅降低 ,潜时正常或轻度延迟 ,P波波形呈“W”波或“U”波为主要特征 ,从而为本病与黄斑疾病和其他视神经疾病的鉴别提供了客观依据。 Objective:To explore the value of pattern visual-evoked potential(P-VEP)in diagnosis of sella turcica region tumour.Method:The authors made an analysis of P-VEP in 20 patients with sella turcica region tumour and made a record of latency and amplitude in N 75 ,P 100 ,N 135 with the spatial frequencies of 60min and 15min in black-white checks stimulation.Results:The rate of the abnormal of P 100 was 95%(38/40),of the decreased amplitude was 50%(20/40),of the prolonged latency was 7.5%(3/40),of the abnormal latency and amplitude was 37.5%(15/40),of the abnormal wave pattern was 70%(28/40),of which the disappeared wave was 22.5%(9/40),and W or U wave accounted for 47.5%(19/40),of the normal wave was 30%(12/40).Conclusion:The rate of abnormal P-VEP in sella turcica region tumour is high,characterized by decreased amplitude,a normal or slightly prolonged latency,and a W or U shaped P wave.This offers an objective distinction between sella turcica region tumour and the macular disease or other optic neuropathy.
