摘要
目的研究甲状腺乳头状癌中Snail蛋白及E-cadherin蛋白的表达及临床意义。方法采用免疫组化S-P法检测64例甲状腺乳头状癌组织和20例甲状腺瘤组织中Snail及E-cadherin蛋白的表达情况,并分析两者与患者临床病理因素之间的关系。结果甲状腺乳头状癌组织中Snail蛋白的阳性率(71.9%)显著高于甲状腺瘤组织(25.0%)(P<0.01),甲状腺乳头状癌组织中E-cadherin蛋白的阳性率(17.2%)显著低于甲状腺瘤组织(85.0%)(P<0.01);Snail及E-cadherin蛋白的表达与TNM分期及有无淋巴结转移密切相关(P均<0.05);甲状腺乳头状癌组织中Snail蛋白与E-cadherin蛋白的表达呈负相关(r=-0.71,P<0.05);Snail蛋白阳性表达者术后1、3、5年生存率明显低于阴性表达者(P均<0.05),E-cadherin蛋白阴性表达者术后1、3、5年生存率明显低于阳性性表达者(P均<0.05)。结论 Snail和E-cadherin蛋白的表达与甲状腺乳头状癌侵袭、转移和预后相关,Snail蛋白表达增强和E-cadherin蛋白表达降低的甲状腺乳头状癌患者预后较差。
Objective To investigate the expressions of Snail and E-cadherin proteins in thyroid papillary carcinoma and their clinical significance. Methods Sixty-four specimens of thyroid papillary carcinoma tissue and 20 specimens of thy- roid adenoma tissue were collected. The expressions of Snail and E-cadberin proteins were detected by immunohistochemical S-P method, and their correlation with clinico-pathological factors was analyzed. Results The positive expression rate of Snail protein in thyroid papillary carcinoma was significantly higher than that in thyroid adenoma (71.9% vs 25.0%, P 〈 0. 01 ). The positive expression rate of E-cadherin protein in thyroid papillary carcinoma was significantly lower than that in thyroid adenoma ( 17.2% vs 85.0% ,P 〈0.01 ). The expressions of Snail and E-cadherin proteins were all significantly related with TNM stage and lymph node metastasis ( all P 〈 0.05 ). There was negative correlation between expressions of Snail protein and E-cadherin protein in thyroid papillary carcinoma tissue( r = -0. 71 ,P 〈0.05 ). The 1-,3-,5-year survival rates in the patients of thyroid papillary carcinoma with Snail protein positive expression were significantly lower than those in pa- tients with Snail protein negative expression ( all P 〈 0.05 ). The 1-, 3-, 5-year survival rates in patients of thyroid papillary carcinoma with E-cadherin protein negative expression were significantly lower than those in E-cadherin positive patients ( all P 〈 0. 05). Conclusion The expressions of Snail and E-cadherin proteins in thyroid papillary carcinoma are related to the tumor invasion, metastasis and prognosis. High expression of Snail protein and low expression of E-cadherin protein are related to the poor prognosis in thyroid papillary carcinoma.
