摘要
目的检测74例胃癌组织中上皮细胞黏附分子(EPCAM)、CD24的表达,探讨其与胃癌临床病理资料之间的关系,预测阳性表达的预后。方法应用免疫组织化学二步法检测74例经手术切除并有明确病理诊断为胃癌标本中EPCAM、CD24的表达。分析74例胃癌临床病理资料。全组患者100%随访,结合表达阳性率对随访结果进行预后分析。结果EPCAM阳性45例,阳性率60.81%;CD24阳性47例,阳性率63.51%。EPCAM+CD24+共40例,阳性率54.05%;EPCAM与肿瘤浸润深度、WHO组织学分型、脏器受侵犯与否有关;CD24与浸润深度、肿瘤位置、分化程度有关;EPCAM+CD24+与浸润深度、肿瘤位置、分化程度、脏器受侵犯与否有关。EPCAM+、CD24+及EPCAM+CD44+患者生存率下降。结论EPCAM、CD24在胃癌组织中表达阳性率高,可作为初筛实验;EPCAM+、CD24+及EPCAM+CD24+均与胃癌浸润深度有关;EPCAM+、CD24+及EPCAM+CD24+生存率下降,死亡率高。
Objective Detect the expressions of epithelial cell adhesion molecule( EPCAM ) and CD24 in 74 gastric cancer patients to analyze the relationship between the expressions of this and the clini- eopathologieal data and predict the prognosis. Methods Detect the expressions of EPCAM and CD24 in 74 gastric cancer patients who took the surgical excision and were pathologically diagnosed as gastric cancer u- sing the immunohistochemistry. Analyze the elinicopathological data and the protein expressions. Results 45 patients ( 60. 81% ) are EPCAM positive; 47 patients ( 63.51% ) are CD24 positive; 40 patients (54. 05% ) are both EPCAM and CD24 positive. EPCAM expression is correlated with depth of tumor infil- tration, WHO histological typing and viscera invasion ; CD24 expression is correlated with the depth of infil- tration,location of the tumor, differentiation grade;EPCAM + , CD24 + expression positive are correlated with depth of infihration,location of the tumor, differentiation grade and viscera invasion (P 〈 0. 05 ). The sur- vival rate declined in EPCAM + ,CD24 + and EPCAM + CD24 + . Conclusion The positive rate of EPCAM, CD24 expression is high in gastric cancer tissue and can be used as primary screening test;EPCAM + and CD24 + expressions are correlated with the depth of infiltration. EPCAM + , CD24 + and EPCAM + CD24 + have lower survival rate and higher mortality rate.
