2型糖尿病患者血小板膜糖蛋白Ⅵ表达与微血管并发症的相关性研究

Relationship between platelet glycoprotein Ⅵ expression and microangiopathy in T2DM patients

摘要 目的探讨T2DM患者血小板膜糖蛋白Ⅵ(GPⅥ)表达水平与血管性血友病因子(vWF)的关系,分析其在T2DM合并微血管并发症中的作用。方法 262例T2DM患者根据眼底镜检查、UAER和肌电图检查分为微血管病变组和无微血管病变组。采用流式细胞仪检测GPⅥ,采用酶联免疫吸附法检测血浆vWF水平,采用逐步线性回归模型对各种微血管并发症的相关危险因素进行分析。结果 T2DM组患者GPⅥ的平均几何荧光强度(GMFI)高于正常对照(NC)组(P<0.05);微血管病变组高于无微血管病变组(P<0.05)。GPⅥ与HbA1c、UAER、vWF呈正相关(r1=0.6945,r2=0.5657,r3=0.5065,P均<0.05)。回归分析显示,GPⅥ是T2DM合并微血管病变的危险因素。结论 GPⅥ在T2DM合并微血管病变患者中表达升高,提示检测GPⅥ表达水平对预测T2DM患者合并微血管病变的风险具有一定价值。 Objective To explore the relationship between the platelet glycoprotein Ⅵ(GPⅥ) expression and von Willebrand factor(vWF) to analyze its effect on the microangiopathy complications in T2DM patients.Methods According to the results of urine albumin excretion rate(UAER),ophthalmoscopy,and electromyography,the 262 T2DM patients were divided into microangiopathy group and non-microangiopathy group.The level of platelet glycoprotein Ⅵ expression was determined by flow cytometry,the level of plasma vWF was detected by enzyme linked immunosorbent assay,and the related risk factors of various microangiopathy complications in the T2DM patients were analyzed by multiple stepwise regression analysis.Results The geometric mean fluorescence intensity(GMFI) of GPⅥ in the T2DM patients was significantly elevated compared with normal controls(P〈0.05),and it was higher in the microangiopathy group than in the non-microangiopathy group(P〈0.05).The level of GPⅥ expression in the T2DM patients was positively related with the levels of HbA1c,UAER,and vWF(r1=0.6945,r2=0.5657,r3=0.5065,all P〈0.05).The multiple stepwise regression analysis showed that GPⅥ was one of risk factors in the presence of microangiopathy complications.Conclusion GPⅥ expression on platelet surface is elevated in the T2DM patients with microangiopathy complications,suggesting that it is significant in predicting the microangiopathy of these patients.
