摘要 目的探讨三维斑点追踪技术在冠心病(coronary heart disease,CHD)患者左心室扭转中的应用价值。方法CHD组62例,对照组41例,采取心尖全容积图像,应用三维超声斑点追踪成像技术测量并比较左心室旋转角度(RA)、旋转率(RR)、扭转角度(TA)、扭转率(TR)。结果 CHD组中左心室曲线形态与对照组大致相似。相应的RA、RR、TA、TR曲线的波幅减低,波形紊乱,CHD组TR与对照组间差异有显著性意义(P<0.05)。结论 3DT可在三维立体空间内研究左室的旋转及扭转运动,从机械力学角度评价CHD患者左室收缩功能减低。 Objective To evaluate clinical value of three-dimensional speckle tracking (3DT) in the left ventricular torsion in patients with coronary heart disease (CHD). Methods Sixty-two patients with CHD ( CHD group) and 41 healthy volunteers ( control group) were enrolled in the study. The left ventricle (LV) rotation angle (RA) , rotation rate (RR) , torsion angle (TA) and torsion rate (TR) in the apical full-volume image were measured and compared by using three-dimensional speckle tracking imaging. Results The left ventricular curve shape in CHD group was similar to that in the control group. The corresponding amplitude of the RA, RR, TA, TR curve reduced, the waveform disordered, and there was a significant difference in TR between the CHD group and control group ( P 〈 0.05 ). Conclusions Left ventricular rotation and torsion in three-dimensional space can be studied by using 3 DT. Therefore, the 3DT is useful to evaluate the left ventricular systolic function in patients with CHD from the point of view of myocardial mechanics.
