两种手术方式在胃癌根治术患者围手术期护理中的比较

Comparison of perioperative nursing in two surgical methods of patients with gastric cancer

目的:观察胃癌患者在开腹和腹腔镜两种胃癌根治术方式中的术中术后情况,探讨两种手术方式的围手术期护理效果。方法:回顾性分析2008年3月～2010年12月我院实施的209例胃癌根治术患者的临床资料,根据手术方式的不同将患者分为开腹组112例和腔镜组97例,比较两组患者术中、术后的情况及术后5 d的每日护理时间。结果:腔镜组胃癌根治术患者的切口长度、术中出血量、住院费用均少于开腹组(P<0.05),手术时间长于开腹组(P<0.05),住院时间、术后5 d每日的护理时间均短于开腹组(P<0.05),术后肛门排气时间、进食流质时间均早于开腹组(P<0.05),术后镇痛评分低于对照组(P<0.05)。结论:腹腔镜下对患者实施胃癌根治术可有效减少患者术中出血量,加快患者术后胃肠功能恢复,减少住院时间及费用,减轻手术疼痛,并减少护士的工作量。

Objective：To observe the condition with gastric cancer patients in laparotomy and laparoscopic surgery in two ways in the intraoperative and post- operative ,to explore the effect of two kinds of surgical perioperative nursing. Methods：A retrospective analysis the clinical data of our hospital 209 cases of gastric patients from March 2008 to December in 2010. According to the surgical methods ,patients were divided into laparotomy group （112 cases） and lap- aroscopic group （97 cases） ,compared the situations of patients in postoperative nursing and after 5 days of daily nursing time. Results：The laparoscopic group of patients with gastric cancer surgery incision length, blood loss, postoperative anal exhaust time, into the liquid food time, hospital stay, hospital cost, postoperative pain score, after 5 days of nursing time were significantly less titan the laparotomy group （ P 〈 0.05 ）. The time of laparotomy group was signifi- cantly less than laparoscopic surgery group（ P 〈 0.05 ）. Conclusion ： Laparoscopic surgery for patients with gastric cancer patients can be effective in reducing blood loss, accelerated postoperative recovery of gastrointestinal function, reduce hospitalization time and costs, reduce surgical pain and reduce the workload of nurses.
