护理干预在肝硬化合并上消化道出血中的应用体会

Application experiences of nursing intervening to cirrhosis combined with upper gastrointestinal hemorrhage

目的探讨护理干预对肝硬化合并上消化道出血的应用体会。方法将2011年3～12月本院收治的68例肝硬化合并上消化道出血患者随机均分为实验组和对照组。所有患者人院后均迅速建立静脉通路，酌情补液扩容、输血、抗休克，一线使用质子泵抑制剂（PPI）、生长抑素及抗菌药物及其他对症支持处理。实验组给予综合护理干预，对照组则无综合护理。对比两组患者的抢救率、满意程度、出血次数。结果护理组临床有效率高于对照组，但对比差异无统计学意义，P〉0．05。两组患者抢救率对比差异无统计学意义，P〉0．05。护理组治疗满意度和m血次数优于对照组．对比差异有统计学意义，P〈0．05。结论综合护理十预可有效提高患者治疗满意度并减少出血次数，提高生活质量，值得临床广泛应用。

Objective To discuss the application experience of nursing intervening to cirrhosis combined with upper gas- trointestinal hemorrhage. Methods Sixty eight cases of patients of cirrhosis combined with upper gastrointestinal hemor- rhage treated in our hospital from March 2011 to December 2011 were randomly divided into the experimental group and the control group. For all the patients, vein access was established rapidly for them and treatments of fluid infusion dilata- tion, transfusion, antishock agent were given depending on the circumstances. Proton pump inhibitors, somatostatin, an- tibiosis drugs and other supportive treatment were also used. Comprehensive nursing intervening was given to the experi- mental group while none was given to the control group. The rescue rate, degree of satisfaction, times of hemorrhage of two groups were compared. Results The clinical effective rate of the nursing group was higher than that of the control group, but there was no statistical significance （P 〉 0.05）. The save rate of two groups had no significant difference （P 〉 0.05）. The treatment degree of satisfaction and times of hemorrhage were superior to those of the control group and the contrast differ- ence was of statistical significance （P 〈 0.05）. Conclusion Comprehensive nursing and intervening measures can effectively improve the patients＂ degree of satisfaction and reduce times of hemorrhage, as well as improve life quality, so it is worth being widely used clinically.
