摘要
目的:观察电针结合椎旁神经阻滞对高位腰椎间盘突出症的临床疗效。方法:将28例高位腰椎间盘突出症患者随机分为电针组和电针结合椎旁神经阻滞组。电针组选T12~L3夹脊、风市、悬钟、丘墟、阿是等穴进行电针治疗;电针结合椎旁神经阻滞组在电针组治疗的基础上结合椎旁神经阻滞治疗。分别于第1次治疗前、第1次治疗结束后和最后1次治疗结束后测定疼痛视觉模拟评分(VAS)以评价患者疼痛程度。结果:第1次治疗结束后,电针组VAS评分与治疗前比较改善不明显,无统计学意义(P>0.05),电针结合椎旁神经阻滞组与治疗前比较明显降低,有统计学意义(P<0.01)。最后1次治疗结束后,2组VAS评分均较治疗前显著降低,有统计学意义(P<0.01),但2组间VAS评分比较,电针结合椎旁神经阻滞组VAS评分低于电针组,有统计学意义(P<0.01)。结论:电针结合椎旁神经阻滞不仅能迅速缓解疼痛,且较单纯电针减轻高位腰椎间盘突出症患者疼痛的速度更快、疗效也更好,无不良反应,安全性高。
Objective: To survey clinical effects of electro-acupuncture and para spinal nerve block in treating upper lumbar intervertebral disc protrusion. Method: Twenty-eight patients were randomly divided into elec- tro-acupuncture group and group of electro-acupuncture combined with para spinal nerve block. Electro-acupuncture group chose T^2-L3 acupoints: Jiaji (EX-B2), Fengshi (GB 31), Xuanzhong (GB 39), Qiuxu (GB 40), Ashi and others; group of electro-acupuncture combined with para spinal nerve block were given with para spinal nerve block besides the acupuncture. VAS was used to evaluate degree of pain before after the first treating and after the last treating. Re- suit: After the first treating, VAS of electro-acupuncture group improved unclearly compared with that before treating and it had no statistical meaning (P〉0.05). Comparison of group of electro-acupuncture and para spinal nerve block before and after treating demonstrated statistical meaning in VAS (P〈0.01). After the last treating, VAS of both groups decreased significantly compared with before treating and it manifested statistical meaning (P〈0.01), and VAS of group electro-acupuncture and para spinal nerve block was less than electro-acupuncture group and the difference showed statistical meaning (P〈0.01). Conclusion: Electro-acupuncture combined with para spinal nerve block not only can relieve pain quickly, but also are effective without adverse reaction and safe.
