骨水泥双极式半髋关节与全髋关节置换治疗高龄股骨颈骨折的比较

Comparison of cemented bipolar hemiarthroplasty and total hip arthroplasty for elderly patients with femoral neck fractures

目的比较骨水泥双极式半髋关节与全髋关节置换治疗高龄股骨颈骨折的治疗效果。方法选取2008年2月～2012年2月于我院接受诊断和治疗的高龄股骨颈患者108例,随机分为半髋关节置换组与全髋关节置换组两组,每组54例,观察比较骨水泥双极式半髋关节与全髋关节置换治疗高龄股骨颈骨折的治疗效果。结果全髋关节置换组的手术时间平均为(110.3±5.2)min,明显长于半髋关节置换组的(82.3±4.6)min(P<0.05);全髋关节置换组的术中出血量平均为(427.8±58.3)mL,明显多于半髋关节置换组的(233.1±35.1)mL(P<0.05);全髋关节置换组下地时间、并发症、疗效评定等方面与半髋关节置换组之间比较,无统计学意义(P>0.05)。结论患者在身体条件允许的情况下优先采取骨水泥双极式全髋关节置换术,患者术前身体状况较差的宜采取骨水泥双极式半髋关节置换术。

Objective To compare the therapeutic effects of cemented bipolar hemiarthroplasty and total hip arthroplasty for elderly patients with femoral neck fractures. Methods A total of 108 patients with femoral neck fractures in our hospital were randomly divided into hemiarthroplasty group and total hip arthroplasty group, each had 54 cases. The therapeutic effects were observed and compared between the two groups. Results The average operation time of total hip arthroplasty group was （110.3±5.2） min, and it was obviously longer than that of hemiarthroplasty group （P 〈 0.05）. The average intraoperative blood loss of total hip arthroplasty group was （427.8±58.3） mL, and it was obviously more than that of hemiarthro plasty group （P 〈 0.05）. The comparison of out-of-bed time, complication and efficacy evaluation between the two groups made no significant difference （P 〉 0.05）. Conclusion Cemented bipolar total hip arthroplasty should be given the priority in the case of better physical condition while cemented bipolar hemiarthroplasty should be selected when the patients had a worse physical condition.
