盐酸右美托咪定在肢体止血带疼痛中的应用

摘要 目的评价盐酸右美托咪定(艾贝宁)在肢体止血带疼痛中的应用效果。方法选择ASAI~Ⅲ级四肢需用止血带手术患者共40例,随机分为艾贝宁组(A组)与芬氟合剂组(B组),每组20例。观察并记录麻醉前后和静脉注药前后血压、心率。疼痛视觉模拟评分(VAS)。结果上止血带后5min、止血带应用1h、止血带放气时、术后6hA组患者VAS评分均明显少于B组,差异均有统计学意义([3.4±1.7)分比(6.6±1.8)分,(6.7±2.4)分比(7.7±3.8)分,(5.8±3.4)分比(6.9±3.5)分,(2.6±0.6)分比(3.8±0.9)分,均P<0.05],两组患者手术前,止血带应用1h,止血带放气时,术后6h两组患者心率及血压变化差异无统计学意义(P>0.05)。结论艾贝宁可安全、有效地用于术中止血带反应。 Objective To evaluate clinical effect of dexmedetomidine for limbs's pain caused by tourniquet. Methods Forty ASA I-III patients who required tourniquet surgery were randomLy divided into dexmedetomidine group (group A) ,Fentanyl and Droperidol mixture group (group B).There were 20 cases in each group. Blood pressure, heart rate, the pain visual analog scores (VAS) were observed and recorded before and after anesthesia and intravenous injection were recorded in surgery. Results The VAS of A group after tourniquet application lh, tourniquet deflated, the postoperative 6h was significantly less than group B, the difference was statistically significant [ ( 3.4 ±1.7) than (6.6± 1.8), (6.7 ±2.4) than (7.7± 3.8), (5.8 ±3.4 points) than ( 6.9± 3.5), (2.6 ±0.6) than(3.8± 0.9),P 〈 0.05](P 〈0.05).The heart rate and blood pressure change of two groups before surgery, tourniquet applicationlh, tourniquet deflation, the postoperative 6h were not statistically significant (P〉0.05).Conclusion Dexmedetomidine can be safely and effectively for intraoperative tourniquet reaction.
