探讨医院药房调剂差错产生的风险因素和预防措施,以期降低调剂差错率,保障公众用药安全。将药房调剂差错以全面质量管理的分类方法,从药剂人员、药品、工作方法、环境、计算机系统五个方面有针对性的进行阐述和分析。调剂差错的发生是一个多因素相互作用的结果。要想降低调配差错率,需从人员、药品、管理、环境和计算机系统各方面去改进,还要注重多部门内外协调,才能达到综合性防范的目的。

Discussing the risk factors of dispensing errors in Outpatient Pharmacy and the precautionary measures, in order to reduce the dispensing error rate and ensure the public drug safety. Analyzing the dispensing error resulting from the per- sonnel, material, process, environment and equipments respectively in TQM Classification means. The dispensing error is the result of the interaction of multi factors. The reducing dispensing error rate not only depend on the improvements in stuff, medicine, management, environment and the computer information system, but also the coordination of the muhidisciplinary.
