椎管减压椎弓根内固定治疗腰椎管狭窄症临床疗效评价

Clinical efficacy evaluation of surgical decompression and transpedicle internal fixation for lumbar spinal stenosis

目的探讨椎管减压椎弓根内固定治疗腰椎管狭窄症的临床疗效。方法对42例腰椎管狭窄症患者进行手术治疗,行全椎板切除减压、椎弓根内固定及椎间和后外侧融合术。采用JOA下腰痛评分方法评估临床疗效,采用SF-36调查问卷评估治疗前后生活质量的变化。结果优26例(61.9%),良9例(21.4%),可5例(11.9%),差2例(4.8%),优良率为83.3%。治疗后患者SF-36各项分值均显著高于治疗前,差别具有统计学意义(P<0.01)。结论对于腰椎管狭窄患者,行全椎板切除减压,椎弓根内固定及椎间和后外侧融合术,可获得满意疗效。

Objective To explore the clinical effect of surgical decompression and transpedicle internal fixation for lumbar spinal stenosis. Methods 42 cases with lumbar spinal stenosis underwent operation treatment, including laminectomy, transpedicle inter- nal fixation and posterolateral fusion. Japanese Orthopedic Association （JOA） and SF-36 were used to evaluate the clinical effect and life quantity before and after treatment. Results The outcomes were excellent in 26 patients （61.9%）, good in 9 cases （21.4%）, fair in 5 cases （11.9%）, poor in 2 cases （4.8%）, with total effective rate of 83.3%. Comparison with pre-treatment, all domains of SF-36 were significantly increased after treatment （P〈O.05）. Conclusion Laminectomy, transpedicle internal fixation and postero- lateral fusion for lumbar spinal stenosis can obtaine a satisfactory effect.
