锁骨钩钢板固定术后并发症的临床分析

目的分析锁骨钩钢板治疗肩锁关节脱位及锁骨远端骨折术后出现的并发症及原因。方法2004年10月至2010年5月，洛阳正骨医院上肢损伤科应用锁骨钩板固定治疗102例RockwoodIII型以上的肩锁关节脱位及RockwoodU型的锁骨远端骨折患者，术后随访12个月，记录出现的各种并发症。结果术后疼痛25例，内固定取出后仍有疼痛5例；肩关节功能障碍6例；内固定取出后再次发生脱位2例；锁骨钩板断裂1例；肩峰应力骨折2例；锁骨远端应力骨折1例；肩袖损伤1例。结论应用锁骨钩板治疗肩锁关节脱位及锁骨远端骨折术后疗效较满意，但仍存在着各种并发症，该方法仍需进一步完善和改进。

Objective To analyze the complications and causes after the operation of the acromioclavicular dislocation and the distal fracture of clavicle with clavicle hook plate. Methods From October 2004 to May 2010, 102 patients with acromioelavieular dislocation and the distal fracture of clavicle were cured by clavicle hook plate. All the patients were followed up for 12 months. The complications were recorded. Results Postoperative pain in 25 cases, functional disturbance of shoulder in 6 eases, relapsing dislocation in 2 cases, breakage of clavicle hook plate in 1 case, fracture of acromion in 2 eases, distal fracture of clavicle in 1 case, rotator cuff injury in 1 case. Conclusions The clavicle hook plate is reasonable and effective internal fixation in the operation of the acromioclavicular dislocation and the distal fracture of clavicle. But postoperative complications are remained. The method need to improve.
