锁定加压接骨板治疗老年患者肱骨近端粉碎性骨折的疗效

Clinical effects of locking pressure plate on elderly patients with proximal humeral comminuted fracture

目的探讨锁定加压接骨板（LCP）治疗老年患者肱骨近端粉碎性骨折的疗效。方法对16例老年肱骨近端粉碎性骨折患者采用切开复位LCP内固定治疗，根据Neer分型Ⅱ型5例，Ⅲ型8例，Ⅳ型3例。术后均定期门诊随访，并评定其疗效。结果16例患者均获随访，随访时间6～18个月，平均13．5个月，骨折均愈合。肩关节功能评定采用Neer评分，优9例，良5例，可2例，优良率为87．5％。X线检查复位结果：解剖复位6例，近似解剖复位6例，3例复位可，1例差。结论使用LCP治疗老年患者肱骨近端粉碎性骨折操作简单、创伤少、固定牢固、血供破坏少、肩关节功能恢复良好，符合目前骨折治疗理念，是一种较理想的治疗方式。

Objective To investigate the clinical effects of elderly patients with proximal humeral comminuted fracture using LCP. Methods Sixteen elderly cases of proximal humeral comminuted fracture were treated by open reduction LCP internal fixation. According to the Neer classification, 5 cases were type Ⅱ, 8 cases were type m, 3 cases were type IV. All patients were followed up regularly,and assessed its curative effect. Results All patients were followed up for 6 - 18 months （ mean 13.5 months）, and all fractures healed. The shoulder function was evaluated according to the Neer score, 9 cases were excellent, 5 cases were good, 2 cases were fair, the satisfactory rate was 87.5%. X-ray reset the results, 6 cases were anatomical reattachment, 6 cases were approximate anatomical reattachment, 3 cases were reattachment fair, and 1 case was poor. Conclusions Treated with LCP for the elderly patients with proximal humeral comminuted fracture are simple operation, less trauma, fixed firmly, blood supply for less damage, shoulder function recover well and consistent with the current treatment philosophy concept, it is an ideal treatment.
