医疗机构用药差错原因及防范措施

Exploration of Reasons for Medication Errors and Preventive Measures in Medical Institutions

目的:减少医疗机构用药差错发生率,提高合理用药水平。方法:从客观和主观因素两方面分析用药差错的原因,并提出相应的防范措施。结果与结论:医疗机构管理的缺陷、药品管理方式落后等是导致用药差错的客观因素;而医师、药师、护理人员执行上的错误,以及患者自身因素等是导致用药差错的主观因素。应采取借鉴并完善用药差错报告系统、建立"非惩罚性"的上报制度、重视药学部门建设等防范措施,以最大程度地减少用药差错发生。

OBJECTIVE： To reduce the incidence of medication error in medical institutions, and to improve rational drug use. METHODS： From both objective and subjective factors, the reasons for medication errors were analyzed, and the corresponding preventive measures were put forward. RESULTS ＆ CONCLUSIONS： The objective factors of medication errors are defects of the administration of medical institutions and backward mode of pharmaceutical management; implementation errors of physicians, pharmacists and nurse, and the patient＇s own factors are subjective reasons of medication errors. We should learn from and improve medication error reporting system, establish a non-punitive reporting system and pay attention to the construction of pharmaceutical sector to farthest reduce the occurrence of medication errors.
