红皮病型银屑病伴发代谢相关性疾病的情况分析

Analyses on Metabolic-related Diseases in Patients with Erythrodermic Psoriasis

目的分析红皮病型银屑病患者伴发代谢相关疾病(MD)的情况。方法对1995年1月-2010年3月在北京大学第三医院皮肤科住院的53例红皮病型银屑病患者伴发代谢相关疾病的临床资料进行回顾性分析,并与同期住院且年龄匹配的62例斑块状银屑病和60例急性荨麻疹患者(对照组)伴发代谢相关性疾病的情况作比较。结果红皮病型银屑病伴发MD、高脂血症(HLP)、腹型肥胖(AO)、代谢综合征(MS)及高尿酸血症(HUA)几率均显著高于对照组;斑块状银屑病伴发MD、高血压(HT)、HLP、AO、MS及HUA也显著高于对照组;男性红皮病型银屑病患者伴发HUA的几率高于对照组及斑块状银屑病组的男性患者,以上差异均有统计学意义(P均<0.05)。结论红皮病型银屑病与斑块状银屑病常伴发HLP、AO、MS及HUA等代谢相关疾病,男性红皮病型银屑病较男性斑块状银屑病更易伴发HUA。尿酸代谢紊乱是否与红皮病型银屑病发病相关还需要进一步研究。

Objective To study the prevalence of metabolic-related diseases in patients with erythrodermic psoriasis. Methods Metabolic-related diseases in 53 patients hospitalized in Peking University, the Third Hospital from Jan 1995 to Mar 2010 were studied, and compared with 62 age matched patients with plaque psoriasis, 60 age matched patients with acute urticaria （control） hospitalized in the same period. Results The eryth rodermic psoriasis patients had a higher prevalence of metabolic diseases （ MD）, hyperlipidemia （ HLP）, abdominal obesity（AO） ,metabolic syndrome（MS） and hyperuricemia（HUA） （ all P 〈 0.05 ）. Patients with plaque psoriasis also had more hypertension （HT）, MD, HLP, AO, MS and HUA than that of control group （all P 〈 0.05 ）. A higher prevalence of HUA was found in male patients with erythrodermic psoriasis（ all P 〈 0.05）. Conclusion Metabolic-related diseases are Common in patients with erythrodermic psoriasis and plaque psoriasis. Male patients with erythrodermic psoriasis had a higher prevalence of HUA than patients with plaque psoriasis, the significance of HUA should be studied further.
