伴雷诺现象的系统性红斑狼疮临床分析

Clinical analysis on systemic lupus erythematosus with Ranaud's phenomenon

目的探讨伴雷诺现象(RP)系统性红斑狼疮(SLE)的临床特征。方法回顾性分析2003年1月-2008年10月天津市长征医院收治的伴RP的SLE患者临床资料,并与同期收治的91例无RP的SLE患者进行比较。结果伴RP的SLE患者出现脱发、指/趾末端血管炎、甲周红斑、肺脏受累及心电图改变的频率较高,抗U1RNP,ds-DNA及抗Sm抗体阳性率也明显增高,且该组患者SLEDAI评分高于对照组,差异均有统计学意义(P均<0.05),而两组患者肾损害、血液系统损害及其他实验室指标检查结果差异无统计学意义(P均>0.05)。结论伴RP的SLE患者更易发生系统损害和免疫学异常,疾病活动性明显,应予以足够重视,注意患者系统受累情况。

Objective To approach the clinical features and laboratory index of systemic lupus erythematous （SLE） patients. Methods The clinical data of 47 SLE patients with Ranaud＇ s phenomenon in Tianjin Long March hospital from January 2003 to October 2008 were analyzed retrospective and compared with 91 SLE patients without Ranaud＇ s phenomenon. Results The incidence of calvities, digit extreme vasculitis, nail erythema, damage of lung and ECG were higher in Ranaud＇ s phenomenon group, the incidence of anti U1RNP antibody, ds-DNA, Anti-Sm were obviously higher than that in the control group, SLEDAI was also higher than control group. There was statistical significance difference in the two groups （all P 〈 0.05 ）, but had no statistical significance with damage of kidney, blood system and other laboratory indexes （ all P 〉 O. 05 ）. Conclusion SLE patients with Ranaud＇ s phenomenon seems to have higher susceptible systemic damage and immunology abnormal disease activity and have many clinical features. We shoule pay more attention to this phenomenon and systemic damage on SLE patients with Ranaud＇ s phenomenon.
