细针穿刺细胞学结合聚合酶链反应在诊断淋巴结结核中的应用 被引量:3

摘要 目的研究细针穿刺细胞学(FNAC)结合聚合酶链反应(PCR)扩增检测结核杆菌TB-DNA的方法对诊断淋巴结结核的应用价值。方法选取162例淋巴结结核病例进行细针穿刺,涂片HE染色和抗酸染色,用细胞学剩余材料或重新穿刺材料进行PCR扩增检测结核分枝杆菌TB-DNA。结果 162例标本中117例TB-DNA阳性,阳性率为72.2%,抗酸染色阳性71例,占43.8%,抗酸染色阳性的病例TB-DNA均阳性,对照组病例全部阴性。162例细胞学涂片,Ⅰ型17例,Ⅱ型67例,Ⅲ型78例,其中TB-DNA阳性率分别为11.8%、70.1%、88.5%,抗酸染色阳性率则分别为0%、41.8%、55.1%。两者阳性率在Ⅱ型Ⅲ型明显高于Ⅰ型(<0.01)。结论 FNAC结合PCR扩增检测TB-DNA的方法在诊断淋巴结结核的应用中,具有敏感而特异,简便而快速的独特优势。
