发疹性微静脉血管瘤六例分析

Clinical and pathological analysis of six cases of eruptive microvenular hemangioma

目的探讨发疹性微静脉血管瘤（eruptivemicrovenularhemangioma，EMVH）的临床及组织病理学特征。方法回顾性分析6例患者的临床及病理资料。结果6例患者均为中青年，平均发病年龄34岁，男女比例均等；特征皮损为突然发生、泛发全身的红褐色丘疹和结节，直径0．1—2．0cm，无自觉症状。组织病理学显示：真皮内增生的胶原纤维束间广泛的分枝状、薄壁小血管增生，界限不清，部分管腔闭塞。血管内皮细胞无异形。免疫组化示：血管内皮细胞CD34、CD31和第八因子相关抗原（F-Ⅷ）阳性。结论首次提出了发疹性微静脉血管瘤的诊断，认识该病有助于早期诊断并避免不必要的治疗。

Objective To analyze the clinical and histopathologic features of eruptive microvenular hemangioma （EMVH）. Methods The clinical and histological findings in six patients with EMVH were retrospectively reviewed. Results The patients were young or middle-aged, with a median age of onset of 34 years. The ratio of males to females was 1 ： 1. Clinically, all the patients experienced a sudden development of widespread, asymptomatic, brunneus, flat-topped papules and plaques measuring from 0.I to 2.0 em in diameter. Histopathologie examination revealed a poorly circumscribed proliferation of small, irregularly branched, thin- walled blood vessels haphazardly arranged between collagen fiber bundles in the dennis. The lumina of the blood vessels were often collapsed. There was no cellular atypia or mitosis. A variable degree of sclerosis was noted in the surrounding stroma. Immunohistochemical study revealed that the endothelial cells stained positive for CD34, CD31, and factor VIH-related antigen. Conclusions The diagnosis of EMVH is proposed, and awareness of this disease may benefit its early diagnosis and avoidance of unnecessary treatment.
