二维斑点追踪技术评价急性心肌梗死患者左室心内外膜下心肌扭转运动 被引量:3

摘要 目的:应用二维斑点追踪技术评价急性心肌梗死(acute myocardial infarction,AMI)患者左室心内、外膜下心肌扭转运动。方法:AMI患者42例及正常对照组30例。分别存储左室短轴基底部及心尖部二维灰阶图像。使用Qlab7.0工作站进行脱机分析,记录左室上述两短轴切面心内、外膜下心肌旋转角度。42例AMI患者根据左室射血分数(LVEF)分为两组:正常组(LVEF≥50%,22例)与降低组(LVEF<50%,20例)。结果:(1)各组间心内膜下心肌扭转角度均大于心外膜下心肌,两者间均存在跨壁扭转角度梯度,但LVEF减低组此梯度减低。(2)LVEF减低组心肌各层面扭转角度均较其余两组减低。LVEF正常组与正常对照组比较,整体扭转角度及心内膜下心肌扭转角度均减低,心外膜扭转角度虽减低,但差异无统计学意义。结论:AMI患者左室整体及心内膜下心肌扭转角度减低,但在LVEF正常组心外膜下心肌扭转功能尚保持正常,随着左心功能进一步减低,LVEF减低组其心外膜下心肌扭转功能亦减低。
