超声引导上臂外周静脉穿刺置管的临床应用

Clinical Application of Ultrasound-guided Peripheral Venous Catheterization in the Upper Arms

目的探讨超声引导下行上臂外周静脉穿刺置管的操作可行性。方法方便性抽样选取嘉兴市妇幼保健医院28例外周静脉穿刺困难的剖宫产产妇,在超声引导下行上臂外周静脉穿刺置管,观察操作时间、次数及并发症。结果所有患者均在超声引导下完成静脉置管,一次穿刺成功25例(89%)、2次成功2例(7%)、3次成功1例(4%),平均操作时间为(4.3±1.9)min。2例患者(7%)发生静脉渗漏,无其他并发症发生。结论超声引导下静脉置管术对外周静脉困难的患者是一项实用和安全的技术,值得临床推广应用。

Objective To explore the clinical feasibility of ultrasound-guided peripheral venous access in the upper arm. Methods Ultrasound-guided puncture and catheterization of peripheral deep vein was performed in 28 parturients with difficult intravenous access scheduled for Cesarean section. The time to catheter placement, number of attempts and complications were measured accordingly. Results Ultrasoundguided Cannulation was successfully performed in all the patients,which accomplished on the first attempt in 25 （89%） ,on the second attempt in 2（70./00） ,and on the third attempt in one patient （4%） ,respectively. The mean time to catheter placement was 4.3± 1.9 min. Infiltration occurred in 2 patients（7. 1%）. No other complication was noticed. Conclusion Ultrasound-guided peripheral venous catheterization is a useful and safe technique for the patients with difficult intravenous access.
