腹腔镜前列腺癌根治术与开放手术的比较

Comparative study of laparoscopic radical prostatectomy and open radical prostatectomy

目的通过腹腔镜前列腺癌根治术(LRP)和开放前列腺癌根治术(ORP)的对照研究,评价LRP在临床的应用价值。方法收集前列腺癌根治术病例44例,其中行LRP15例、ORP29例。比较两组间手术疗效及安全性。结果 LRP组15例,成功完成14例。患者平均手术时间为(271.21±109.20)min,平均出血量(395.57±310.07)ml。ORP组29例患者平均手术时间(154.73±64.95)min,平均出血量(746.62±604.55)ml。两组出血量和手术时间比较,差异均有统计学意义(t分别=4.39、2.04,P均<0.05);而两组住院时间和术后血清前列腺特异抗原(PSA)比较,差异均无统计学意义(t分别=1.98、1.27,P均>0.05)。结论 LRP在出血的控制方面优于ORP,LRP是安全可行的。

Objective To evaluate clinical value of LRP through comparing the open radical prostateetomy （ORP） and the laparoseopie radical prostatectomy （LRP）. Methods A total of 44 patients who underwent radical prostatectomy were chosen. Operation effect and security between 15 cases of LRP and 29 cases of ORP were compared. Results Of 15 patients underwent LRP, 1 ease converted to open surgery. Mean operative time was （271.21±109.20）min, mean estimated blood loss was（395.57±310.07）ml. 29 patients underwent O RP, Mean operative time was （154.73 ±64.95）rain, mean esti- mated blood loss was （746.62±604.55）ml. The blood loss of LRP group was less than that of the ORP group but operation time was longer（t=4.39, 2.04,P〈0.05）. Length of hospital stay and post operation PSA had no significantly differences （t= 1.98,1.27,P〉0.05）. Conclusions LRP is better than ORP in controlling blood loss, and it is safe and feasible.
