血清降钙素原在急性细菌性眼内炎诊断中的临床意义

Discussion of clinical influence of serum procalcitonin in acute bacterial endophthalmitis

目的探讨血清降钙素原(PCT)在急性细菌性眼内炎诊断中的临床意义。方法 36例(36只眼)临床诊断为急性细菌性眼内炎的患者在玻璃体切割手术前和术后7 d接受PCT测定和外周血白细胞计数,分析PCT水平和急性细菌性眼内炎诊断之间的相关性。结果 34例患者PCT检测阴性,2例伴发高热、白细胞计数显著升高的病例出现PCT阳性结果,经过治疗后感染控制PCT恢复阴性。所有病例治疗前后的白细胞计数改变有统计学显著性差异。结论眼部感染引发全身感染时PCT测定可呈现阳性反应,对急性细菌性眼内炎病例进行PCT检测可有助于排除由眼内炎伴随全身严重感染的可能性。

Objective To discuss the clinical influence of serum procalcitonin in acute bacterial endophthRlmitls. Methods The PCT level, and WBC counting in 36 cases of clinical diagnosed acute bacterial endophthalmifis were tested prior to and 7 days after vitrectomy surgery. The correlation of PCT and diagnosis of acute bacterial endophthaimitis was analysed. Results Positive PCT case was found in 2 eases with high fever and high white blood cell count. After the infection was under control, PCT negative was present in the two cases. PET negative was found in the other 34 cases. WBC counting change was statistically significant prior to and after treatment. Conclusim PCT positive presents in cases with systemic infection caused by ocular infection. Testing of PCT level can help to rule out systemic infection secondary to bacterial endophthslmitis.
