低分子肝素钠对单侧输尿管梗阻大鼠肾间质纤维化的预防及机制探讨 被引量:2

摘要 目的观察低分子肝素钠对单侧输尿管梗阻大鼠肾间质纤维化的预防作用,并探讨其作用机制。方法将24只大鼠随机分为假手术组(Sham组)、模型组(UUO组)、治疗组(LMWH组)各8只。UUO组及LMWH组行左侧输尿管结扎术,Sham组只游离但不结扎和剪断输尿管。LMWH组术后皮下注射低分子肝素钠4 mg/kg 1次/d,Sham及UUO组每天皮下注射等量生理盐水。14 d后处死大鼠,取梗阻侧肾制作标本。行HE及Masso染色,光镜下观察肾小管间质病变;采用免疫组化ABC法检测肾小管间质中的基质金属蛋白酶9(MMP-9)、MMP抑制因子1(TIMP-1)、转化生长因子-β1(TGF-β1)、Ⅳ型胶原(COL-Ⅳ)。结果 Sham组肾间质损伤评分为(0.18±0.10)分,UUO组为(2.06±0.44)分,LMWH组为(1.05±0.14)分;UUO组、LMWH组与Sham组比较,P均<0.01;LMWH组与UUO组比较,P<0.01。Sham组肾小管间质中MMP-9、TIMP-1、TGF-β1、COL-Ⅳ的积分光密度值分别为1.27±0.92、0.00、2.67±0.35、0.90±0.24,UUO组分别为5.35±1.12、11.00±1.94、7.41±0.86、12.17±2.63,LMWH组分别为11.62±1.62、6.74±1.71、5.70±1.03、6.40±2.04;UUO组、LMWH组与Sham组比较,P均<0.01;LMWH组与UUO组比较,P<0.01。结论低分子肝素钠可减轻肾间质纤维化程度,与其上调肾小管间质中MMP-9表达、下调TIMP-1、TGF-β1、COL-Ⅳ表达有关。
