弥漫性轴索损伤常规CT及MRI检查阴性的FLAIR或DWI诊断 被引量:6

摘要 目的探讨常规CT、MRI检查阴性的弥漫性轴索损伤(DAI)的诊断。方法回顾性分析18例常规CT、MRI检查阴性的DAI患者影像学资料。结果常规CT以及MRI自旋回波序列T1WI、快速自旋回波序列T2WI检查未见明显异常。经液体衰减反转恢复序列(FLAIR)和/或弥散加权序列(DWI)检查显示14例胼胝体、基底节区存在散在的、斑点状高信号影。18例患者采用综合治疗后按GOS评价预后,恢复良好13例,中重残5例。结论常规CT和MRI检查对DAI的敏感性有限,FLAIR、DWI序列提高了DAI诊断率,应作为DAI的重要检查方法,但目前的检查技术仍有一定局限性,部分DAI患者仍可能难以通过影像学明确诊断。
