18F-FDGPET/CT在淋巴瘤放射治疗计划中的应用

摘要 目的:18F-FDGPET/CT可提高淋巴瘤诊断的敏感性和特异性,成为淋巴瘤诊疗的新标准。本研究的目的是检验融合PET/CT在制定淋巴瘤治疗计划方面的应用。方法:回顾性分析我院2009年3月至2011年9月收治的16例PET阳性的患者,其中,10例为非霍奇金淋巴瘤,6例为霍奇金淋巴瘤。每例患者均由一名放射肿瘤医师在CT和PET/CT上单独勾画GTV。所有患者的治疗计划均由CT和PET/CT勾画靶区体积(PTV)来制定。用剂量-体积直方图评价正常组织剂量和PTV覆盖的范围。结果:以CT图像勾画靶区体积制定的治疗计划为参照,共评价了16个计划,PET的应用改变了12个靶点的体积(75%)。8个靶点的PTV增大了(50%),中位数为11%(6-40%);4个靶点的PTV缩小了(25%),中位数20%(6-75%)。3个患者CT图像上勾画的PTV不能完全覆盖PET/CT勾画的PTV,重新制定了治疗计划。结论:FDG-PET应用到基于CT图像的治疗计划后,明显改变了一部分淋巴瘤患者的治疗方案设计、靶区体积勾画和正常组织的受照剂量。
