RVG数字化X线成像与透明牙技术对下颌第一前磨牙根管形态的比较研究 被引量:4

摘要 目的:利用RVG(Radio Visio Graphy)数字成像与透明牙技术研究下颌第一前磨牙根管的解剖形态,比较2种方法对根管形态判读的一致性。方法:收集湖北中部地区拔除的下颌第一前磨牙105例,采用RVG数字成像和透明牙技术显示根管系统形貌,以Vertucci分类法进行根管类型分型,采用Kappa统计法对两种方法结果的一致性进行分析。结果:透明牙技术发现Ⅰ型70个(66.7%),Ⅲ型4个(3.9%),Ⅴ型24个(22.8%),Ⅵ型2个(1.9%),Ⅶ1个(0.9%),1-2-3型2个(1.9%),1-2-4型1个(0.9%),罕见类型1个(0.9%);RVG法发现Ⅰ型76个(73.3%),Ⅲ型3个(1.9%),Ⅴ型22个(19.1%),Ⅵ型1个(0.9%),Ⅶ1个(0.9%)。两种方法观察同时判读单根管和多根管的比率为64.8%和20%,分型结果间Kappa值为0.58,为中度一致,而两种方法观察根尖分歧、根尖分叉、根管侧支和管间吻合无一致性。结论:RVG数字图像能反应下颌第一前磨牙根管形态的主要容貌,但对复杂根管系统及细微结构根尖分歧、根尖分叉、根管侧支和管间交通等显示远不及透明牙,RVG法和透明牙法对下颌第一前磨牙根管系统的显示有差异,临床诊疗切不可过于依靠RVG。
