摘要
目的:探讨2型糖尿病患者的血脂水平与其糖化血红蛋白水平、胰岛素抵抗情况及胰岛β细胞功能的关系。方法:入选2型糖尿病患者136例,分别以糖化血红蛋白10.0%、胰岛素抵抗指数2.8以及胰岛功能指数20为界值将其分为高糖化血红蛋白组(HH组,n=76)、低糖化血红蛋白组(LH组,n=60);胰岛素重抵抗组(IR组,n=65)、胰岛素轻抵抗组(SR组,n=71);胰岛β细胞功能轻减退组(BN组,n=74)和胰岛β细胞功能重减退组(BW组,n=62),分析各组的血脂水平。结果:HH组和LH组的三酰甘油(TG)和高密度脂蛋白(HDL)的水平均无显著差异(P>0.05),HH组的总胆固醇(TC)和低密度脂蛋白(LDL)的水平显著高于LH组(P<0.05);IR组的TG水平显著高于SR组(P<0.05);BW组的HDL水平显著高于BN组(P<0.05)。结论:2型糖尿病患者血糖越高、胰岛素抵抗越严重、胰岛β细胞功能越差,血脂紊乱越严重。
Objective:To investigate the relationship between the level of blood lipid and glycosylated hemoglobin, insulin re- sistance,β cell function in patients with type 2 diabetes. Methods: A total of 136 hospitalized patients with type 2 diabetes have been enrolled in the trial. They were divided into high glycosylated hemoglobin group (Group HH) and low glycosylated hemo- globin group (Group LH) with the cutoff point of 10.0% ,insulin high-resistance group (Group IR) and insulin low-risistance (Group SR) with the cutoff point of 2.8, low-hypo ~ cell function group (Group BN) and high-hypo β cell function group (Group BW) with the cutoff point of 20. The levels of blood lipid between each pair of groups were compared respectively. Re- suits: The levels of total cholesterol (TC) and low density lipoprotein cholesterol (LDL) Group HH were significantly higher than those in Group LH (P〈0.05). The level of triglyceride (TG) in Group IR was significantly higher than that in the Droup SR (P〈0.05). The level of high density lipoprotein cholesterol (HDL) in Group BW was significantly higher than that in Group BN (P〈0.05). Conclusions: Poor glucose control, highly insulin resistance and low β cell function were correlated with the disorder of blood lipid in patients with type 2 diabetes.
