初诊甲状腺功能亢进症病人心理健康水平及其与应付方式的相关性

PATIENTS WITH PRELIMINARY DIAGNOSIS OF HYPERTHYROIDISM: THEIR MENTAL HEALTH LEVEL AND COPING STYLE

目的研究初诊甲状腺功能亢进症(甲亢)病人心理健康水平及其与应付方式的相关性。方法收集我院门诊初诊甲亢病人76例,应用症状自评量表(SCL-90)、应付方式问卷(CSQ)进行调查,评估心理健康水平和应付方式类型,并进行相关分析。结果初诊甲亢病人SCL-90总分及各因子分均高于全国常模,不同应付方式类型病人SCL-90评分差异有显著性(F=3.07、17.31,P<0.05、0.01)。有心理问题组病人自责、幻想、退避3个维度得分高于心理健康组,差异有显著性(t=-5.94～-1.70,P<0.05)。SCL-90总分与自责、幻想、退避、合理化维度呈正相关(r=0.33～0.69,P<0.01)。进入多元逐步回归方程的变量有自责、幻想、解决问题(F=31.52,P<0.01,b=-0.16～0.47,P<0.05)。结论初诊甲亢病人心理健康水平低于正常人,故在临床应用药物治疗甲亢的同时还应进行心理教育以改善应付方式。

Objective The investigate the mental health situation in patients with newly diagnosed hyperthyroidism and their style of coping with the condition.Methods Seventy-six patients with newly diagnosis of hyperthyroidism were enrolled.SCL-90 and Coping Style Questionnaire（CSQ）,were applied to investigate and assess their mental health condition and coping style,a correlation analysis was done as well.Results The total score of SCL-90 and factor score in the patients were higher than national norms.The score of SCL-90 was significantly different among patients with different types of coping style（F=3.07,17.31;P〈0.05,0.01）.The scores of self-blame,illusion,and retreat were higher in the patients who had mental problem than that in mental healthy ones,the difference being significant（t=-5.94——1.70,P〈0.05）.Score of SCL-90 was positively related with self-blame,fantasy,avoidance and rationalization（r=0.33-0.69,P〈0.01）.The variables entering multiple stepwise regression equation were self-blame,fantasy,problem solving（F=31.52,P〈0.01,b=-0.16-0.47,P〈0.05）.ConclusionThe level of mental health in newly diagnosed patients with hyperthyroidism is lower than that in normal person.Therefore,du-ring the drug treatment of hyperthyroidism,psychophylaxis should be carried out at the same time.
