骨基质明胶复合红骨髓及同种骨修复节段性骨缺损的实验研究 被引量:33

The use of BMG- autologous red marrow- allograft in the repair of segmental radius defects in rabbits

摘要 目的 探讨骨质基明胶 (BMG)复合自体红骨髓移植及其与同种库存骨联合移植修复骨缺损的效果,为其应用于临床提供实验依据。方法 将 BMG与自体红骨髓复合,制成 BM;并采用兔桡骨干 1.5 cm缺损的动物模型。通过观察大体形态及进行 X线检查、组织学检查等,比较 BM移植、 BM与同种库存骨联合移植 (简称 BMA移植 )及自体骨移植修复节段性骨缺损的疗效。结果 (1)BMA移植,术后 16周全部骨愈合 (5/5),修复效果优于自体骨和 BM移植 (P 0.05); (3)BMA、 BM、自体骨移植明显优于对照组 Obejective To investigate the efficacy of combined use of BMG- autologous red marrow- allograft (BMA) in repairing segmental bony defects. Methods A combined grafting material (BM)was made by BMG and autologous red marrow. BM, BMA or autograft was used respectively to repair 1.5 cm segmental defects created in the radius of rabbits. The repair capability for each of the treatment was assessed physically, radiographically and histogically at 2,4,8,12,16 weeks after operation. Results 1) 100% (5/5) of the specimens had complete bony union with trabecullar and cortical bone on the sites with BMA at 16 weeks after implantation, and BMA exhibited the strongest defect- repair power. 2) BM grafts achieved a better bone formation than autografts at the early stage of the repair process(2 and 4 weeks after implantation), and no significant union score was discovered between BM and autograft at 8, 12, 16 weeks. 3) Histological examination showed that new bone formation in the defect sites treated with BMA and BM first appeared early at 2 week, which could not be found in the saline control group. Conclusion BMA, BM or autograft implantation can efficiently repair bony defect, and the capability of BMA is the best of all by the way of satisfactory osteoconduction, osteoinduction and osteogenesis of bone marrow. Bone marrow- related bone formation is inclined to be an early phenomenon.
