植物粪石性急性小肠梗阻CT诊断价值

THE VALUE OF CT IN THE DIAGNOSIS OF ACUTE SMALL INTESTINE OBSTRUCTION DUE TO PHYTOBEZOARS

目的探讨植物粪石性急性小肠梗阻的CT表现及诊断价值。方法回顾性分析经外科手术证实的7例植物粪石性急性小肠梗阻病人的临床资料及CT表现。结果 7例小肠粪石均为单发,其中2例同时伴有胃内粪石。在CT影像上,小肠粪石呈边缘清楚的卵圆形或圆形、其内密度不均或夹有小气泡影的软组织包块影,其中空肠3例,回肠4例,均表现为近段小肠扩张,管径>3cm,远段小肠突然萎陷。结论小肠植物粪石在CT上具有特征性表现,CT检查是诊断植物粪石性急性小肠梗阻的有效方法。

Objective To observe the CT manifestations of phytobezoars in acute small intestine obstruction（SIO） and assess its diagnostic value.Methods A retrospective review was done for seven patients with acute SIO caused by phytobezoars confirmed surgically.Their clinical records and CT manifestations were analyzed.Results Of the seven cases,all the fecalith in the small intestine were solitary,of which,two with simultaneous fecalith in the stomach.On CT image,the fecalith in the intestine appeared as round or oval in shape with well defined,and,internally,uneven density or soft-tissue masses fixed with gas bubbles could bee seen,of which,three were found in jejunum,four in ileum,all showing dilatation of proximal intestine（caliber3 cm） and distal part collapsed suddenly.Conclusion There are mark manifestations of phytobezoars in small intestine on CT,which is an effective method in the diagnosis of this condition.
