深冻异体肌腱重建膝关节后内侧角及内侧副韧带的临床研究 被引量:2

摘要 目的探讨深冻异体肌腱重建膝关节后内侧角及内侧副韧带的临床疗效。方法以41例长期膝关节内侧不稳定的患者为研究对象,术前MR检查示内侧副韧带Ⅲ°损伤,查体外翻应力试验阳性,膝关节旋转不稳定,所有患者行手术治疗。术后1年应用IKDC评分、Lyshlom评分评价临床效果;采用30°及0°外翻应力试验和外翻应力位X线片评估关节稳定性。结果所有患者术后IKDC评分、Lyshlom评分较术前均有明显改善。外翻应力位X线测量内侧关节增宽间隙由术前(10.4±2.1)mm减小到术后(2.6±0.7)mm,其中37例患者<3 mm,4例患者3~5 mm。术后1年30°外翻应力试验阳性4例(19.76%),0°阳性3例(7.32%),旋转不稳阳性4例(9.76%),关节活动度术前(133.1°±7.1°)与术后(130.7°±5.9°)比较差异无统计学意义(P>0.05)。7例患者术后1年仍有膝内侧局限性压痛。结论异体肌腱重建膝关节后内侧角及内侧副韧带能明显改善膝关节外翻及旋转稳定性,短期临床疗效确切。
