改良双Endobutton技术治疗Tossy Ⅲ型肩锁关节脱位

Improvement technique in the treatment of type Ⅲ acromioclavicular joint dislocation with double Endobutton plate

目的介绍一种治疗新鲜肩锁关节Ⅲ度脱位的新技术,并探讨改良双Endobutton技术治疗TossyⅢ型肩锁关节脱位的疗效。方法 2008年1月～2012年1月收治28例TossyⅢ型肩锁关节脱位,采用双Endobutton技术重建喙锁韧带的功能。其中男16例,女12例;年龄24～51岁,平均(33.6±11.2)岁;采用改良双Endobutton技术,手术时间在伤后1～7 d,平均(3.5±2.3)d,术后1周开始功能锻练。对患者进行跟踪随访,评定疗效。结果 28例患者均得到随访,时间4～18月,平均6.4月,脱位全部复位,无再脱位。均恢复正常生活和工作。按照Karlsson标准评定患者术后的肩关节功能恢复情况:A级21例,B级7例。结论改良双Endobutton技术创伤小,固定牢固,操作方便,是TossyⅢ型肩锁关节脱位理想的治疗方法。

Objective To introduce a new surgical method for treatment of Tossy type Ⅲ acromioclavicular joint dislocation and explore the clinical outcomes of improvement technique in the treatment of type m acromioclavicu-lar joint dislocation with double endobutton plate. Methods From January 2008 to January 2012, 28 patients with Tossy m acromioclavicular joint dislocation were treated with improvement technique of double Endobutton plate . The Tossy m acromioclavicular joint dislocation was reconstructed coracoclaviacular ligament with double ENDOBUTTON technology. There were 16 males and 12 females, with an average age of 33.6 ：t： 11.2 years old ranging from 24 to 51 years. The time from injury to operation was 1 to 7 days （means 3.5 ＋ 2.3 days ）. All patients were followed and the clinical outcomes were analyzed. Results The 28 patients were followed up from 4 to 18 months （averaged 6.4 months ）. All acromioclavicular joint dislocation was reduced. All patients recovered from abnormal function and no complication happened. According to the scales of Karlsson, the post - operation functionl of shoulder - joint were A grade in 21 patients and were B grade in 7 patients. Conclusion The improvement technique of double Endobutton plate is an effective ideal treatment methods for Tossy Ⅲ acromioclavicular joint dislocation basing on its less invasion and convenience and good biocompatibility.
