乌司他丁对重型颅脑损伤患者血清S-100B蛋白及NSE的影响 被引量:7

摘要 目的探讨不同剂量乌司他丁对重型颅脑损伤(sTBI)患者血清S-100B蛋白及NSE的影响。方法选取2010年8月-2011年8月收治的27例伤后1d内入院的sTBI患者为研究对象,以乌司他丁治疗剂量的不同随机分成3组:小剂量组(n=8)每日予乌司他丁10万单位静推,大剂量组(n=7)每日予乌司他丁40万单位静推,2组用法及疗程均为每6h1次,连续5d;对照组(n=12)未予乌司他丁治疗。各组患者于伤后第1~5d抽取伤侧或重伤侧颈静脉球血,采用酶联免疫吸附法检测血清中S-100B蛋白及NSE含量。结果在伤后第3,5d,大剂量乌司他丁组血清S-100B蛋白及NSE浓度显著低于对照组(P<0.05)。结论使用40万单位乌司他丁可以降低重型颅脑外伤患者血清S-100B蛋白及NSE水平,在伤后早期对神经有保护作用。 Objective To explore the therapeutic effects of different dosages of ulinastatin on serum S-100B protein and NSE in patients with severe traumatic brain injury.MethodsTwenty seven patients diagnosed severe traumatic brain injury,whose pre-hospital time was 24 hours,were enrolled from Aug.2010 to Aug.2011.The patients were randomly divided into three groups by different dosages of ulinastatin.The patients in low dosage group(n=8) were used 100 thousand units of ulinastatin per 6 hours from the 1st to 5th day in hospital;The patients in high dosage group(n=7) were injected 400 thousand units of ulinastatin and the patients in the control group(n=12) were only under the basic treatment.Blood samples were taken via retrograded jugular blub cannula to compare the concentration of serum S-100B protein and NSE by enzyme linked immunosorbent assay.ResultsCompared with the control group,the concentration of serum S-100B protein and NSE in high dosage group were decreased greatly in the 3th,5th days after sTBI(P〈0.05).ConclusionUlinastatin by using with 400 thousand units per 6 hours one day could decrease the concentration of serum S-100B protein,NSE from the patients with severe traumatic brain injury.It might be playing neurological protective role during the early post-traumatic period.
