颈总动脉压迫试验在经颅多普勒检查中的应用价值

摘要 目的探讨经颅多普勒检查中应用颈总动脉压迫试验(简称"压颈试验")的必要性。方法选择1000例行常规颅内血管检查的脑梗塞患者,对比分析进行压颈试验与未进行压颈试验的结果。结果轻度狭窄血管834条,误诊29例。中、重度狭窄血管922条,颈内动脉终末段狭窄误诊21例,大脑中动脉闭塞误诊4例,大脑后动脉狭窄误诊29例,颈内动脉颅外段病变误诊8例,眼窗探测颈内动脉虹吸段狭窄误诊9例。总误诊率约7.1%。结论经颅多普勒医生应该掌握并熟练应用压迫颈总动脉试验,该试验可确认检测到的血流信号、判断血流来源、确定患者侧枝循环代偿功能。 Objective To explore the necessity of common carotid artery compression during the examina- tion of the transcranial Doppler. Mehods 1000 patients with stenosis disease of cerebral and carotid artery were per- formed transcranial Doppler, and the results were compared with common carotid artery compression and wihout it. Results The rate of misdiagnosis was higher in the patients without common carotid artery compression. The func- tion of giant intracranial aneuryms could not be defined. Conclusions Transcranial Doppler with common carotid ar- tery commpression should be controlled and applied skillfully, which can provide an efficient approach to observe blood supply, detect blood signal and intracranial collaterals.
