摘要
[目的]了解年龄相关性白内障患者的睡眠状况及其影响因素。[方法]应用匹兹堡睡眠质量指数问卷(PSQI)和自行设计的调查表,对2009年4月至2010年12月入住巨野县中医院的280例年龄相关性白内障患者进行调查,采用多因素非条件Logistic回归分析睡眠质量的影响因素。[结果]调查280例,睡眠质量差者133例,占47.50%。睡眠质量差者所占比例,已婚者为40.44%,单身/丧偶者为76.36%(P<0.01);经济独立者为35.12%,经济不独立者为66.07%(P<0.01);双眼较好视力轻度下降者为36.61%,中度下降者为45.54%,重度下降者为68.66%(P<0.01)。调查的280例中,睡眠质量有问题的占33.57%,入睡时间有问题的占51.07%,睡眠时间有问题的占30.00%,睡眠效率有问题的占45.71%,睡眠障碍有问题的占20.71%,催眠药物有问题的占4.29%,日间功能有问题的占10.36%。多因素非条件Logistic回归分析结果,已婚者发生睡眠质量较差的危险性下降(OR为0.245);重度视力下降、冠心病、吸烟均是睡眠质量的危险因素(OR值分别为2.968、1.726、1.616)。[结论]将近一半的年龄相关性白内障患者睡眠质量较差,良好的婚姻状况有益于睡眠,重度视力下降、冠心病、吸烟可使睡眠质量下降。
[Objective] To understand the sleep condition and influence factors of patients with age-related cataract. [Methods]A total of 280 patients who had been admitted to the traditional Chinese medicine hospital of Juye county during April 2009-December 2010 with age-related cataract were interviewed using Pittsburgh sleep quality index (PSQI) and a self-designed questionnaire. The influence factors of sleep condition were obtained by multiple non-conditional logistic re- gression models. [Results]There were 47.50% (133/280) cases suffering from worse sleeping among which 40.44% and 76.36% cases were the married and the unmarried/loss of spouse( P 〈0.01) 335.12% and 66.07% were economically in- dependent and economically dependent( P 〈0.01); 36.61 %,45.54 % and 68.66 % were slight visual loss, medium visual toss and severe visual loss of the better eye respectively( P 〈0.01). The proportion of 280 cases with a problem of subjec- tive sleep quality was 33.57%, and the corresponding proportion for sleep latency, sleep duration, habitual sleep efficiency, sleep disturbances, use of sleep medications, and daytime dysfunction was 51.07%, 30.00%, 45.71 %, 20.71 %, 4.29% and 10.36 %. The risk of worse sleeping in the married decreased by multiple non-conditional logistic regression models ( OR = 0. 245) ;severe visual loss, coronary heart disease and smoking may he risk factors of sleep quality( OR = 2. 968, OR 1. 726 and OR = 1. 616,respectively). [Conclusion]Nearly half patients with age-related cataract have worse sleeping. A good marriage status is beneficial to sleeping,but severe visual loss, coronary heart disease and smoking could decrease the sleep quality significantly.
