萎缩性胃炎患者幽门螺杆菌感染与血清胃蛋白酶原亚型水平变化的相关性研究

Studies on relationship and clinical value of the pepsinogen subtypes and chronic atrophic gastritis

目的:探讨血清胃蛋白酶原亚型(PG-Ⅰ,PG-Ⅱ)水平和PG-Ⅰ/PG-Ⅱ比值对慢性萎缩性胃炎(CAG)的临床应用价值。方法:使用酶联免疫吸附实验(ELISA)检测196例CAG患者和35例正常人血清PG-Ⅰ和PG-Ⅱ水平及PG-Ⅰ/PG-Ⅱ比值的变化。结果:CAG组血清PG-Ⅰ水平明显低于正常对照组,差异有统计学意义(P<0.05)。血清PG-Ⅰ水平随病情进展明显降低,各组间比较差异有统计学意义(P<0.05)。PG-Ⅰ/PG-Ⅱ比值的变化与血清PG-Ⅰ水平变化呈正相关。CAG组血清PG-Ⅱ水平略高于正常对照组,差异无统计学意义(P>0.05)。幽门螺杆菌(Hp)阳性CAG组与Hp阴性CAG组血清PG-Ⅰ和PG-Ⅱ水平及PG-Ⅰ/PG-Ⅱ比值基本一致,统计分析,差异无统计学意义(P>0.05)。结论:血清PG-Ⅰ和PG-Ⅰ/PG-Ⅱ比值的降低,是辅助诊断CAG的一项较好的血清学指标,适于人群筛查及胃镜检查禁忌者。

Objective ：To explore the clinical effect of pepsinogen - I （PG - I ）,pepsinogen - （PG - ~ ） and PG - I /PG - K in serum of patients with chronic atrophic gastritis（CAG）. Methods .. The levels of PG - I and PG - in serum of 196 cases of CAG and 35 cases of normal controls were measured by enzyme-linked immunosorbent assay （ELISA）. Results ..The serum PG - I levels of CAG group of patients were lower notable compared with that o~ the normal control group and with significant difference（P^0. 05）. The levels of PG - I in serum of patients with CAG were decreased remarkably alone with disease progression and among the three groups of CAG with significant differ- ences（P^0. 05）. PG - I /PG - I ratio changes with levels of serum PG - I were positively correlated. The serum PG - lI levels of patients with CAG slightly higher than the normal control group and with no significant difference（P~ 0.05）. The levels of serum PG - I ,PG - 1T and PG - I /PG - ]~ ratio in patients with CAG of helicobacter pylori（Hp）- positive and Hp-negative group were roughly consistent and with no significant differense（P〉0.05）. Conclusion .. The decreasing of PG - I in serum and the ratio of PG - I /PG - 11 are a better serological markers suitable for population screening and endoscopy contraindications.
