成人先天性胆总管囊肿诊断和手术治疗分析

Analyses of operation diagnosis and treat on congenital choledochal cyst

目的探讨先天性胆总管囊肿的诊断和治疗方法。方法 2004.03-2010.06收治的15例先天性胆总管囊肿患儿的诊疗经过进行回顾分析。结果本组病例中15例患者中,行囊肿切除+肝总管空肠Roux-Y吻合13例,2例行囊肿外引流,术后半年行二期肝总管或胆总管空肠Roux-Y吻合术,均痊愈出院。术后随访无一例死亡或发生严重并发症,术后病理结果证实均为先天性胆总管囊肿。结论 B超是首选的检查手段,必要时行CT和MRCP检查。本病一旦确诊,应尽早手术,手术以囊肿切除+肝总管空肠Roux-Y吻合术为首选术式。

Objective To evaluate the diagnosis of congenital bile duct cysts and treatment.Methods 2004.03-2010.06 15 patients admitted to the diagnosis and treatment of congenital choledochal cyst in children after were analyzed.Results The cases of 15 patients,hepatic duct cyst excision ＋ Roux-Y jejunal anastomosis in 13 cases,2 cases were cyst drainage,six months after Phase II hepatic duct or common bile duct line jejunal Roux-Y anastomosis,were cured.No case of death or postoperative follow-up of serious complications,postoperative pathological findings were confirmed choledochal cyst.Conclusion B is the preferred inspection means super,if necessary,row CT and MRCP examination.Once the diagnosis of the disease,should surgery,surgery to hepatic duct cyst excision ＋ Roux-Y jejunal anastomosis as the preferred surgical.
