2型糖尿病患者血清维生素D_3水平和胰岛素抵抗及胰岛B细胞功能的相关性研究 被引量:17

摘要 目的观察2型糖尿病患者血清25-羟维生素D3[25(OH)D3]水平和胰岛B细胞功能及胰岛素抵抗的关系。方法选取确诊2型糖尿病(T2DM)患者50例,非糖尿病对照组46例,用酶联免疫吸附法(ELISA)测定所有入组者血清中25(OH)D3含量。用稳态模型胰岛素抵抗指数(HOMA-IR)、胰岛素敏感指数(IAI)、胰岛B细胞功能指数(HBCI)和空腹胰岛B细胞功能指数(FBCI),评估胰岛B细胞功能及胰岛素抵抗,分析T2DM患者25(OH)D3水平和上述参数之间的相关性。结果 (1)T2DM组血清25(OH)D3水平明显低于非糖尿病对照组(P<0.01);(2)T2DM组患者25(OH)D3和IR均呈正相关和IAI呈负相关。25(OH)D3水平与FBCI、HBCI无关(P>0.05)。线性回归分析提示25(OH)D3是IR和IAI的影响因素。结论T2DM患者体内VD水平降低,可能是其胰岛素抵抗的原因之一。补充VD可能有助于糖尿病的预防和治疗。 Objective To explore the relationship between serum vitamin D level and the islet B cell function as well as insulin resistance in the patients with type 2 diabetes mellitus. Methods The 25-hydroxy-vitamin D3 levels were measured in fifty patients with T2DM and forty-six healthy subjects, respectively. Insulin secretion was evaluated by HOMA-IR and IAI, and insulin resistance was evaluated by FBCI and HBCI. The linear regression analy- ses were conducted between vitamin D level and these indexes, respectively. Results The serum vitamin D levels were lower in the patents with T2DM than those in the healthy subjects. The vitamin D level in the patents with T2DM was positively correlated with HOMA-IR, and negatively correlated with IAI. There was no association between vitamin D level and FBCI or HBCI. Conclusions The serum vitamin D insufficiency may play a role in the pathogenesis of insulin resistance. Vitamin D supplementation may be helpful in the treatment of T2DM.
