经阴道子宫肌瘤剔除术在基层医院的应用体会及临床资料分析——附36例报告

Application Experience and Clinical Analysis of Transvaginal Myomectomy in Basic Hospital(A Report of 36 Cases)

目的探讨经阴道子宫肌瘤剔除术(TVM)在基层医院开展的适应症及可行性。方法回顾分析我院2009年1月-2011年12月对36例子宫肌瘤患者行经阴道子宫肌瘤剔除术,观察其手术时间、术中出血、术后疼痛、肛门排气时间、术后病率及术后住院时间。结果 36例TVM均获成功,手术所需平均时间为96±48min,出血量平均为138±71.6ml,术后肠功能恢复肛门排气平均时间为21.45±3.75h,术后住院时间平均为7.80±1.36d。术后原有症状均消失,B超复查未发现子宫肌瘤。结论正确掌握手术适应证,TVM在基层医院开展具有创伤小、恢复快、无需复杂设备,是一种安全可行的子宫肌瘤剔除的微创手术方法。

Objective To evaluate the feasibility and effectiveness of transvaginal myomectomy（TVM） in primary hospitals.Methods From January 2009 to December 2011,the data of 36 patients with hysteromyoma underwent transvaginal myomectomy was researched with retrospective analysis,and the operation time,intraoperative bleeding,postoperative pain,exhaust time,postoperative morbidity and postoperative length of hospital stay were observed.Results All of 36 cases were performed byTVM successfully.An average of operation time was 96±48 min,intraoperative bleeding volume was 138±71.6 ml,anal exhaust time was 21.45±3.75 h in recovery of intestinal function after operation,postoperative length of hospital stay was 7.80±1.36 d.All patients had a significant recovery clinical findings,without hysteromyoma reexamined by B ultrasound.Conclusion TVM has the advantage of slight trauma,early recovery and no need of complicated equipment if the operation indication is restrictedly controlled.So this minimally invasive surgery is appropriate to promote the application of the primary hospitals.
