摘要
目的了解哺乳期妇女服用200 mg碘油胶丸的有效保护期,并探索其尿碘变化的规律。方法在新疆阿克苏地区库车县牙哈镇的4个村选择109名近半年内未服用过碘油丸的19~36岁哺乳期妇女及其子女(20天~18个月婴幼儿),采用现场干预的方法,并于服药后的1、3、7、14、30、50、75、100 d分别采集尿样(5ml以上),同时采集婴幼儿实验组及对照组尿样,测定尿碘值。结果口服200 mg碘油胶丸后,哺乳期妇女及其婴幼儿尿碘中位数在干预后第一天达到峰值,为1 399.42μg/L、1 383.94μg/L,之后开始下降到干预后50 d的140.24μe/L、261.42μg/L;1~7 d维持高值段,7~30 d尿碘值下降较快,30 d以后尿碘缓慢下降;1~30 d干预组大于对照组,差异有统计学意义(P<0.01,非参数wilcoxon秩和检验),50d后两组间差异没有统计学意义(P>0.05)。结论哺乳期妇女口服200mg碘油胶丸的尿碘最长有效保护期不到50 d。
Objective To understand effective protection of 200 mg dose of iodized oil pills taken by lactating women, and to explore their variation pattern of urinary iodine. Methods At four villages of Yaha Town of Kuche County, Aksu, 109 lactating women and their children (20-day to 18-month infants) without taking iodized oil pills within half a year were se- lected to carry out on-site intervention investigation. Urine samples (above 5 ml) of the women were collected after taking the pills 1, 3, 7, 14, 30, 50, 75, 100 days, respectively, and urine samples of their infants in experimental group and control group were also collected to detect for urinary iodine. Results After taking the pills, median of urinary iodine of the lactat- ing women and their children reached peak on the first day of intervention with the value of 1 399.42 ~tg/L and 1 383.94 gg/ L. And then, it began to decline to 140.24 gg/L and 261.42 gg/L after intervention for 50 days. It kept higher during 1 day to 7 days, decreased rapidly in period of 7 to 30 days and descended slowly after 30 days, with intervention group more than the control during 1 day to 30 days. The difference was of a statistical significance (P〈0.01), and no statistical significance was found between the two groups after 50 days (P〉0.05). Conelusions The longest effective protection of 200 mg iodized oil pills taken orally by the lactating women is less than 50 days.
